Photo / Video Image Release Agreement

WALK PRODUCTIONS INC.

By proceeding with my online photo submission, I understand and agree that my image / name / likeness / photo may be used in perpetuity throughout the world in all media including, but not limited to, print, television, electronic media, and the internet.

I hereby acknowledge by proceeding with this submission and with knowledge that Walk Productions Inc. it’s subsidiaries, successors, assigns, and/or licensees intend to act in reliance hereon, I irrevocably grant Walk Productions, subsidiaries, successors, assigns, and licensees the absolute full right, power, authority and permission to copyright, use, advertise, publish and distribute my name, likeness, photograph, video image, in any media form and/or product.

My name and/or likeness may be used for broadcast, editorial, testimonial art, promotional or any other lawful purpose whatsoever without restriction.

I hereby waive any and all rights that I have to inspect and/or approve the finished project or media copy that may be used in connection herewith or the use to which it may be applied.

I hereby state I am 18 years of age or older as of the date hereof, or this form bears the signature of my parent or legal guardian, thereby releasing Walk Productions it’s subsidiaries, successors, assigns and or licensees from any and all liability resulting there from.
